Things you will need for registration
Please see enrollment forms below. Bring the completed forms along with any required documents when registering your child.
- Name of Student
- Address
- Date of Birth
- Report Card
- What school are they coming from?
- Do they have an IEP? If yes, submit an updated IEP
- Bring in all required documents. (By appointment only)
- All updated Immunization records
